I have completed my first attempt at putting the Willow Creek parent observation questionnaires into a full well check template. I resisted the urge to add other elements to the HPI simply for the sake of consistency; I did format the ROS, PE, Diagnoses and Plans to what I thought would be typical for a 2 week visit, although the exam may be a little long.
Please give it a go and tell me what you think.
You can find it under my username after you choose Select Template. It is called "WCP- 2 weeks".
If you want to actually import the template, send me an e-mail lash.kevin@gmail.com and I can attach it to my reply. It will eventually end up in Tank Template or some other place that's easy to find.
